Registered Nurse – Home Health
Schedule: 8 hours
Assignment Details:

Contract Length: 12 weeks
Guaranteed Hours: 40
Requirements:

• Current state license as a Registered Nurse
• 1-2 years experience as a Registered Nurse in a clinical care setting
• 2+ years Home health experience
• Homecare Homebase Software Experience
• OASIS Experience
• Proof of current CPR certification
• Current driver's license
• Must have reliable transportation and insurance

Assess patients in home health and hospice to identify their physical, psychosocial, and environmental needs.
Develop, document, and evaluate the Plan of Care to maintain quality and continuity of care.
Educate patients and their families regarding care, medications, and home safety.
Supervise and provide clinical direction to home health aides to ensure quality service delivery.
Monitor assigned cases to ensure compliance with third-party payer requirements.
Serve as a liaison between patients, families, and the healthcare team.
