Founded in 1977 as the Senior Care Action Network, SCAN began with a simple but radical idea: that older adults deserve to stay healthy and independent. Manages programs and leads clinicians providing primary care within integrated care delivery model to SCAN members.

Manages the performance of clinical teams to achieve defined clinical care outcomes.

Oversees clinicians in daily operations, ensuring adherence to ethics and quality standards in the delivery of patient care by Advanced Practice Providers (AAP). Selects, on-boards, trains and develops clinicians within the integrated care delivery model.

Provides clinical care to include performing exams, ordering diagnostic and screening tests, developing plans of care, accurately documenting patient medical history and care, and educating and consulting with patients and families.

Collaborates with senior management on the resolution of operational and performance issues and the development of new plans and programs. Improves and integrates APP functions and systems across specialties and departments, including the development and implementation of APP Best Practice Models.

Networks and builds relationships with internal and external key stakeholders to promote membership growth and quality outcomes for integrated care delivery products.

Maintains professional and technical knowledge by attending educational workshops; establishing personal networks; Maintains client confidence and protects operations by keeping information confidential.

Contributes to team effort by identifying and achieving goals/targets as needed.

Supervises/Manages Others.

Master’s degree from an accredited school of nursing.

• Oregon RN, NP, and Furnishing licensure required. Strong strategic thinking skills to facilitate the development and delivery of new clinical products and services to seniors and the community.
