Position Overview The Licensed Practical Nurse (LPN) provides direct resident care , observes and monitors resident health and vitals, maintains documentation, and collaborates with Registered Nurses (RNs) and Unit Managers on care plans and medical histories.

Deliver direct personal care to residents

Monitor and document resident health, vitals, and changes in condition

Collaborate with RNs and Unit Managers on care planning and medical histories

Maintain compliance with company policies, procedures, and state regulations

Contribute to a positive, collaborative, and professional care environment

Licensed Practical Nurse (LPN) license in Massachusetts

Long-term care experience preferred but not required

Strong communication, teamwork, and compassionate care skills

Dedication to providing high-quality, resident-centered care
