In this role, you will manage a caseload of patients, complete comprehensive assessments, and coordinate care across the interdisciplinary team. You will play a key role in supporting patients throughout their home health episode while promoting independence, safety, and positive clinical outcomes.## Qualifications**Qualifications:*** Graduate of an accredited Diploma, Associate, or Bachelor’s degree in Nursing* Current Registered Nurse (RN) license in the state of Texas* Minimum 2 years of RN clinical experience; home health experience preferred* Strong knowledge of home health regulations, OASIS documentation, and care planning* Experience completing Start of Care (SOC), Recertifications, and Resumption of Care visits preferred* Ability to manage a patient caseload and work independently in a field-based environment* Experience coordinating care with interdisciplinary teams (PT, OT, ST, MSW, HHA)* Strong clinical assessment, documentation, and critical thinking skills* Excellent communication and interpersonal skills with patients, families, and physicians* Reliable transportation with valid driver’s license and auto insurance* Proficiency with EMR systems (HomeCare HomeBase preferred)# **Essential Functions:*** Manage and coordinate all aspects of home health patient care and case management* Perform Start of Care (SOC), Recertification, and Resumption of Care assessments* Develop, implement, and update individualized plans of care based on patient needs* Coordinate care with physicians and interdisciplinary team members to ensure continuity of care* Supervise and provide clinical direction to LVNs and Home Health Aides* Lead and participate in interdisciplinary case conferences (IDG/IDT)* Monitor patient progress and adjust care plans to achieve desired outcomes* Provide patient and caregiver education on disease management, medications, and home safety* Complete accurate and timely clinical documentation in compliance with regulatory standards* Coordinate discharge planning and ensure smooth transitions of care* Monitor visit utilization, productivity, and patient outcomes* Maintain compliance with agency policies, procedures, and state and federal regulations**Benefits:**• Highly competitive compensation • Generous 401K with matching contributions • Continuing education reimbursement for professional growth • Ample vacation, sick leave, and holidays. • Over-productivity pay • Employee recognition program with financial rewards • Mileage reimbursement • Comprehensive medical, dental, and supplemental insurance***We are an Equal opportunity employer; We offer an excellent benefit plan to include 401K with match, CEU reimbursement, vacation, sick, holidays, medical, dental, and supplemental insurance Plans as well as a Highly competitive compensation package.****
