Job Title Nurse Practitioner, Advanced Practice Provider

Role Overview The purpose of a Nurse Practitioner at Oak Street Health is to provide effective and equitable value‑based primary care to adults on Medicare, helping keep them healthy and avoiding hospital stays. Nurse Practitioners see patients independently and collaborate with physicians, delivering longitudinal care and building patient relationships.

Core Responsibilities Provide exceptional primary care and conduct office visits for routine and acute issues.

Administer annual wellness visits and health risk assessments with a focus on thorough, accurate documentation.

Coordinate care with other providers, specialists, testing facilities, and agencies.

Lead population health initiatives in collaboration with the Care Team (e.g., ensuring all eligible females receive evidence‑based breast cancer screening every two years).

Assist the care team with phone triage and outreach.

Educate patients on health conditions, care plans, and treatments.

Participate in Oak Street Health promotional activities.

Required Qualifications Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.

Family Nurse Practitioner, Adult‑Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner.

Active, non‑probationary state Nurse Practitioner license.

Preferred Qualifications Experience in primary care, such as internal medicine, geriatrics, or family medicine.

Understanding of HCC (Hierarchical Condition Categories) documentation, ICD‑10 coding, and Health Risk Assessments.

Desire to be part of an innovative model focused on empirically‑guided population health.

Bilingual proficiency in applicable areas.
