Description

Conduct thorough evaluations of patients in both inpatient and outpatient environments and develop personalized treatment plans.
Implement physical therapy interventions with a focus on enhancing mobility, strength, balance, and overall function.
Collaborate closely with nursing staff, doctors, case managers, and other rehabilitation specialists to ensure optimal care outcomes.
Guide patients and their families through therapeutic objectives, discharge strategies, and home-based exercises.
Maintain complete and accurate documentation of evaluations, treatments, and patient progress in accordance with facility and regulatory standards.
Engage in interdisciplinary team discussions and care team rounds to facilitate comprehensive care delivery.
Education
~ Doctorate in Physical Therapy (DPT) or a similar degree from an accredited institution.

Certifications
Valid state licensure as a Physical Therapist .
CPR certification is preferred.
Specialized certifications (e.g., in neurology or geriatrics) are beneficial but not required.
