As an Admissions RN, you'll play a vital role in welcoming patients into home health services by completing comprehensive Start of Care assessments and developing individualized care plans that set them up for success.

Complete 1–3 Start of Care (SOC) visits each day throughout Monmouth, Middlesex, Somerset, and Union Counties
Develop individualized plans of care in collaboration with physicians and the interdisciplinary care team
Deliver skilled nursing care, including medication administration, wound care, treatments, and patient education
Ensure documentation is accurate, timely, and compliant with regulatory standards
Current Registered Nurse (RN) license in the State of New Jersey
Minimum of one year of nursing experience (Home Health experience preferred)
Experience completing OASIS documentation and Start of Care visits strongly preferred
Comfortable using electronic medical records (EMR); Homecare Homebase (HCHB) experience is a plus
Passion for providing exceptional, patient-centered home health care
