COTA - Occupational Therapy

Trinity Health PACE Alexandria is seeking a compassionate and skilled Certified Occupational Therapy Assistant (COTA) to support our mission of keeping older adults living safely and independently in their homes and community.

Full time

Under the supervision of a licensed Occupational Therapist, the Certified Occupational Therapy Assistant (COTA) provides occupational therapy services to PACE participants to support independence, safety, and quality of life. Services are provided in the PACE center, clinic, participant homes, and community settings in coordination with the Interdisciplinary Team (IDT).

Implements occupational therapy treatment plans as established by the supervising Occupational Therapist

Provides therapy services to frail elderly participants in clinical, home, and community environments

Trains participants and caregivers on adaptive equipment, safety strategies, and energy conservation

Observes, reports, and documents participant progress and functional changes

Participates in interdisciplinary team meetings and care planning

Completes documentation in a timely manner in accordance with CMS PACE regulations, Trinity Health policies, and program standards

Graduate of an accredited Occupational Therapy Assistant program

Current Louisiana OTA/COTA license in good standing (or eligible)

Experience in geriatrics, PACE, home health, or community‑based care

Ability to travel locally between PACE sites and participant homes
