Physical therapist - health - therapy

Position Summary We are seeking a licensed Physical Therapist (PT) to provide home health therapy services across our growing coverage zones. Placentia, Anaheim, Fullerton, Brea Costa Mesa, Newport Beach, Huntington Beach, Fountin Valley, Santa Ana Key Responsibilities Conduct comprehensive initial assessments, developments, and implementation of individualized physical therapy plans of care. Provide functional mobility training, therapeutic exercise instruction, gait training, balance training, and pain management techniques in patients' homes. Document clinical notes timely and accurately using our home health electronic medical records (EMR) software. Collaborate effectively with physician teams and our internal clinical coordinators to ensure seamless continuity of care. Qualifications & Requirements Active, unencumbered license as a Physical Therapist (PT) in the state of California. Graduate of an accredited Physical Therapy program. Prior experience in Home Health Care is highly preferred (experience with OASIS documentation is a plus, but we are open to training the right clinical candidate). Valid California Driver’s License, reliable transportation, and active auto insurance.
