Overview Job Title: Home Health Admissions RN, Light Full Time
What You’ll Do Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that supports you.
Conduct comprehensive start-of-care assessments, including clinical evaluations, medication reconciliation, and development of initial care plans aligned with physician orders.
Educate and guide patients and families through the admission process, setting expectations for care, reviewing diagnoses and treatment plans, and ensuring they feel informed and supported from day one.
Apply strong clinical judgment and critical thinking to coordinate admissions efficiently, address barriers to care, and ensure accurate, timely, and compliant EMR documentation.
travel to patient homes within the assigned territory.
