Physical Therapist - PT - Rehabilitation Therapy - (Physical Therapy )

The Physical Therapist will provide comprehensive therapy services to patients in a home health setting. Utilizing clinical expertise, the therapist will evaluate patient needs, develop treatment plans, and implement therapeutic techniques to improve mobility and quality of life.
Responsibilities Conduct initial evaluations and develop individualized treatment plans.
Implement therapy interventions to enhance patient function and mobility.
Collaborate with interdisciplinary teams to optimize patient care.
Document patient progress and adjust treatment plans as necessary.
Educate patients and their families regarding rehabilitation strategies.
Required Experience / Certifications / Licensure Graduate degree from an accredited Physical Therapy Program (approved by the APTA).
Minimum of one-year experience in physical therapy, preferably in home health or facility settings.
Current and unrestricted Physical Therapy Licensure.
Current CPR certification.
