Part Time RN Advocate (Flexible Hours)

Our Client is looking for a Patient Advocate to join their team servicing Chicago and North Chicago area ASAP !
This is a part-time, hourly role $60/hr; Clinical Advocacy & Care Coordination
Coordinate care across healthcare systems to ensure continuity and quality.
Maintain accurate medication lists and client medical profiles.
Support hospital discharges and transitions of care.
Identify and resolve barriers to care.
Attend medical appointments and translate medical information.
Reinforce care plans and promote informed decision-making.
Support client autonomy while respecting personal values.
Liaise with providers, facilities, and community resources.
Document all activities accurately and timely in electronic record.
Maintain accurate billing and reporting.
Ensure HIPAA compliance and strict confidentiality.
Maintain active Illinois RN license.
Maintain or pursue Board Certification in Patient Advocacy.
Active Illinois Registered Nurse (RN) license.
Minimum 3 years experience in geriatrics, care coordination, case management, community nursing, OR chronic disease management.
Psychiatric experience a plus.
Reliable transportation for local client visits.
Competitive hourly rate based on experience and credentials.
Flexible scheduling based on client needs.
