Licensed Practical Nurse (LPN) PRN 2nd and 3rd Shift

Provide hands-on nursing care in accordance with scope of practice
Collaborate with RNs and physicians during rounds and care planning
Coordinate care and communicate changes to residents' families and team members
Review and implement physician orders; assist in emergencies, including CPR
Document all care accurately and timely in the medical record
Complete admission documentation and assist in discharge planning
Ensure safe, clean, and well-stocked medication and treatment carts
Support audits and assist with quality improvement efforts
Current LPN license (in good standing)
Excellent communication skills with residents, families, and staff
Calm under pressure and able to act decisively in emergencies
A team-focused, resident-first mindset
Dependability and punctuality for scheduled shifts
Flexible scheduling: Day, Evening, NOC, and PRN shifts available for both 8 or 12 hours shifts!
Graduate of accredited LPN School of Nursing.
Current valid LPN license in the state employed.
Six months experience in a long-term care environment preferred.
