Compensation: Estimated Annual Compensation: $70,000 – $86,000. This position is paid on a per-visit basis. The compensation reflected is an estimate of annual earnings for full-time status.

Per-Visit Pay Structure:

Single visit type structure

Minimum productivity expectation: 32 visits per week

Example annual earnings: 32 visits per week × $36 per visit × 52 weeks = $59,904

Ability to increase visit volume and earnings beyond the minimum requirement

No cap on earning potential

Position Overview

Austin, TX is seeking a compassionate and dependable Licensed Vocational Nurse (LVN) to support patients in a home health setting. This role is ideal for nursing professionals who enjoy working independently while delivering personalized, high-quality care directly in patients’ homes. The LVN will collaborate closely with an interdisciplinary healthcare team to promote positive patient outcomes and continuity of care.

Key Responsibilities

Conduct skilled nursing visits in patients’ homes in accordance with physician orders and individualized care plans

Monitor, assess, and evaluate patient conditions, including chronic disease management and post-acute recovery

Accurately document assessments, interventions, and outcomes in the electronic medical record

Communicate and collaborate with interdisciplinary healthcare team members to coordinate patient care

Educate patients and families on medications, care plans, and lifestyle modifications

Travel to patient homes within the assigned coverage area while maintaining effective time management

Participate in on-call rotation as required

Qualifications

Graduate of an accredited practical or vocational nursing program

Minimum of one (1) year of clinical nursing experience

Home health experience preferred, but not required

Licensure and Certifications

Active Licensed Vocational Nurse (LVN) license in the State of Texas

Current CPR or BLS certification

Valid driver’s license

Reliable transportation and current auto insurance

Pay and Schedule Details

Per-visit pay model based on visit type and complexity

Typical schedule: Monday through Friday, 8:00 AM – 5:00 PM, with some flexibility

Minimum expectation of 32 visits per week
