Del Corazon Hospice is a compassionate, patient-centered hospice organization dedicated to providing comfort, dignity, and support to patients and families during life's most meaningful moments. Our team is made up of professionals who truly care and take pride in serving our community through exceptional end-of-life care.
We are seeking a Hospice Certified Nursing Assistant (CNA) to serve patients primarily in Santa Fe and Espanola , providing care in Skilled Nursing Facilities (SNFs), Assisted Livings and patients' homes . This role requires compassionate hands-on care, excellent communication, and a heart for hospice.
Provide personal care and emotional support to hospice patients in SNFs and private residences
Maintain accurate documentation per hospice and regulatory guidelines
Travel throughout the service area daily
Hospice or home health experience preferred
Valid driver's license, reliable transportation, and auto insurance
Ability to travel daily within service area
Mileage reimbursement
Ongoing training and professional development
Day shift Monday-Friday
