Registered Nurse (RN) - Home Health
- Schedule: Monday-Friday Day Schedule + On-Call Rotation
Join a nonprofit home health agency that focuses on delivering skilled, patient-centered care within the community. This in-person home health position in Mesa County, CO offers:
- Flexibility in structuring your day
- Meaningful one-on-one patient care
Active Registered Nurse (RN) License in State of Colorado (or active NLC compact license)
- Valid driver's license, reliable transportation, and auto insurance
- Ability to work independently and manage a structured caseload
- Home health experience preferred, but not required
Develop and update individualized plans of care
- Provide skilled nursing care and appropriate interventions
- Monitor patient progress and report changes to the care team
- Educate patients and families on medications, treatments, and care plans
- Coordinate care with LPNs, therapists, caregivers, and providers
- Participate in the on-call rotation
