Registered Nurse Hospice Liaison

Schedule: Full-Time

The VNA Health Group is seeking a compassionate and experienced Registered Nurse Hospice Liaison to join our dedicated Hospice Team. This vital role focuses on assessing patients for hospice eligibility, coordinating their transition to hospice care, and conducting patient admissions. The Registered Nurse Hospice Liaison will be responsible for covering the following hospitals: Newark Beth Israel Medical Center, Clara Maass Medical Center, Cooperman Barnabas Medical Center, and Jersey City Medical Center. As a frontline advocate for hospice services, you will foster continuity, collaboration, and trust among patients, families, and referring partnersall while supporting our mission of delivering patient- and family-centered care.

Serve as a bridge between healthcare providers and hospice patients to ensure compassionate, coordinated transitions of care

Conduct in-home and facility-based hospice admissions with clinical accuracy and sensitivity

Engage with the community and clinical partners to increase access and awareness of hospice care

Collaborate with a team of interdisciplinary professionals committed to quality and comfort

Support the organization's mission by striving for excellence and fostering positive interpersonal relationships

Assess patient needs and provide counseling to patients and families during the hospice referral and admission process

Conduct comprehensive hospice admissions, including clinical assessments, eligibility verification, and documentation

Coordinate transition to hospice services to promote continuity of care

Establish and maintain liaison with physicians, external partners, and internal care teams

Monitor and support quality and appropriateness of services and programs

Conduct community outreach to enhance hospice awareness and accessibility

Assist in the development and implementation of a hospital discharge program at Community Medical Center

Adhere to agency infection control and safety protocols and organizational policies

Active New Jersey RN license (BSN preferred)

Excellent clinical skills with experience in hospice and/or community health nursing

Experience conducting hospice or homecare admissions strongly preferred

Excellent organizational, customer service, and interpersonal skills

Strong computer proficiency, including MS Office Suite

Valid NJ driver's license, personal vehicle, and proof of insurance

Working Conditions/Physical Demand: Fast-paced healthcare environment with frequent phone, scanner, email, and computer use. Requires use of fine motor hand movement and local travel via personal automobile.

In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $43.00 to USD $51.00.
