Hispanic Alliance for Career Enhancement
The Hispanic Alliance for Career Enhancement is seeking a dedicated RN to facilitate transitions for members from nursing facilities back into the community. This vital role requires managing referrals, coordinating safe discharges, and ensuring member satisfaction through outpatient follow-ups.
