Your role will be onsite in our state-of-the-art outpatient office facility located in Jacksonville, Florida. Here, you will collaborate closely with a multidisciplinary team to deliver effective rehabilitation services. You will have access to modern rehabilitation technology and tools that facilitate improved treatment outcomes.

Develop and implement individualized physical therapy treatment plans aimed at preventing disability and relieving pain, while enhancing clients' mobility and functional abilities.

Supervise and guide physical therapy assistants to ensure optimal execution of treatment plans and adherence to best practices.

Analyze clients' home environments and provide recommendations to facilitate a successful rehabilitation process within their daily living spaces.

Deliver physical therapy interventions directly in accordance with physician prescriptions, which may include therapeutic exercises, manual therapy, and modalities such as traction and massage.

Participate in ongoing assessment and modification of treatment plans based on client progress, documenting clinical records including evaluations, progress notes, and discharge summaries.

Actively engage with interdisciplinary teams during case conferences, contributing insights and suggestions to refine comprehensive care strategies for patients.

Instruct clients and their families on the effective use of therapy devices and self-care techniques to promote independence in their rehabilitation journeys.

Maintain compliance with all legal and ethical standards while providing timely documentation and administrative support for clinical and payroll processes.

Your primary focus will be working with a diverse array of clients, including those recovering from orthopedic surgeries, sports injuries, neurological disorders, and chronic pain conditions. Our patient-centered approach ensures that each individual receives personalized care that respects their unique needs, preferences, and goals. You will play a critical role in empowering patients to regain their independence and improve their quality of life.

A Doctorate in Physical Therapy (DPT) from an accredited program.

Active and unrestricted Physical Therapist license in the State of Florida, demonstrating adherence to professional standards with no history of sanctions.

Proficiency in a variety of manual therapy techniques and the ability to design effective therapeutic exercise programs tailored to individual client needs.

Experience using Electronic Medical Records (EMR) systems, specifically Web PT or similar software, to maintain accurate patient documentation.

Strong knowledge of current physical therapy methodologies supported by ongoing research and clinical best practices.

Experience working in outpatient rehabilitation settings, especially with diverse patient populations.

Ability to collaborate effectively with interdisciplinary healthcare teams and communicate regularly with clients and their families.
