RN Home Health Care Nurse

You will communicate regularly with physicians, therapists, social workers, and other team members to ensure coordinated, patient-centered care.A typical day in this role includes visiting multiple patients in their homes, documenting care and progress using an electronic medical record system, and adjusting care plans based on clinical findings.
