BrightSpring Health Services is seeking a Physical Therapist for a Home Health assignment in Ashland, KY. This position offers the opportunity to provide one-on-one patient care in the home setting while working independently throughout the community. The Physical Therapist will evaluate patients, develop individualized treatment plans, coordinate care with the interdisciplinary team, and help patients improve mobility, function, and independence.
Complete evaluations, reassessments, and discharge planning
Provide skilled physical therapy services in the home environment
Collaborate with physicians, nurses, caregivers, and therapy staff
Participate in interdisciplinary care planning
Active Kentucky Physical Therapist License
Home Health experience preferred
