As a Care Team Nurse, you'll serve as a clinical leader and trusted resource for patients, families, and colleagues. Providing telephone and in-office triage for pediatric patients
Assessing physical, emotional, and psychosocial needs of children and families
Educating patients and families on health conditions, treatments, and preventive care
Coordinating care for patients with chronic conditions and complex healthcare needs
Supporting patients after emergency room visits and hospital discharges
Collaborating closely with pediatricians, nurse practitioners, medical assistants, and administrative staff
Maintaining accurate documentation within our electronic medical record system
Helping ensure smooth daily clinic operations and exceptional patient experiences
Current Massachusetts RN or LPN license
Graduate of an accredited nursing program
Commitment to providing compassionate, family-centered pediatric care
Experience in pediatrics preferred, but passionate nurses with transferable skills are encouraged to apply
