The Home Health Aide is a full-time, on-site role based in Prosper, Frisco & Plano, TX. This role provides direct, hands-on support to clients in their homes, including assistance with personal care such as bathing, grooming, dressing, toileting, and mobility. The Home Health Aide helps with light housekeeping, meal preparation, laundry, and may support medication reminders as directed by licensed clinicians or care plans. Daily responsibilities include monitoring clients’ well-being, documenting care activities, and promptly reporting changes in condition to supervisors or nursing staff. The role also involves offering companionship, promoting safety and comfort, and collaborating with the care team to follow individualized care plans and uphold client dignity.

Demonstrated skills in personal care assistance, including bathing, grooming, dressing, toileting, and safe mobility support.

Ability to perform basic home support tasks such as meal preparation, light housekeeping, laundry, and maintaining a safe environment.

Strong interpersonal, communication, and active listening skills to build trust, provide companionship, and follow care instructions accurately.

Experience following individualized care plans, documenting services provided, and reporting changes in client condition to clinical staff.

Capacity to work respectfully with diverse populations, including seniors, veterans, and individuals with disabilities or special needs.

High school diploma or equivalent; completion of a Home Health Aide, CNA, or related caregiver training program preferred.

CPR and First Aid certification strongly preferred.

Reliable transportation to client homes in the Plano and greater Dallas area and ability to work assigned full-time schedules.
