Inova Fairfax Hospital is looking for a dedicated Registered Nurse (RN) Patient Care Navigator - Outpatient Heart Failure to join the team. This role will be full‑time day shift: Monday‑Friday 8:00 a.m. - must be flexible). Travel may be required - rotating to our Oakville & Mark Center locations in Alexandria, in addition to Woodbridge and Loudoun. Sign‑on bonus and relocation assistance eligible.

Coordinates a patient's plan of care which may include appointment scheduling, confirming patient's attendance, transportation, assisting with referral process, and answering patient questions.

Acts as a liaison, advocate and point of contact to patients in need of services such as screening, diagnostic treatment, clinical research participation, support services, and follow‑up/aftercare plan by helping them access the best healthcare outcomes from doctors, hospitals and others who provide their required services.

Refers cases to the appropriate resources for assistance. Conducts comprehensive assessment of the patient's health and psychosocial needs and develops a case management plan in collaboration with the patient and their support system.

Educates patient, family or caregiver(s), and members of the health care delivery team regarding treatment options, community resources, insurance benefits, self‑care management, etc. Collaborates with multidisciplinary teams to address patient questions and responses about the care process and treatment options.

Develops/maintains reliable systems to document, track and monitor patient data.

Bachelors Degree in Nursing

Experience: 3 years of nursing experience
