Hospice Registered Nurse

Meaningful, one-on-one patient care focused on comfort and quality of life

• Autonomy with support – manage your caseload with a collaborative interdisciplinary team
• Strong team partnership with physicians, social workers, chaplains, and aides
• As a Hospice RN at VitalCaring, you are more than a nurse - you are the clinical leader, advocate, and steady guide for your patients and their families.

You'll lead the plan of care, manage symptoms, and ensure every patient receives personalized, comfort-focused care aligned with their goals and wishes. Lead Patient-Centered Hospice Care

• Develop and manage individualized, comfort-focused care plans
• Deliver skilled nursing care with a focus on pain and symptom management

Serve as the Care Coordinator

• Act as case manager, coordinating care across the interdisciplinary team
• Collaborate with physicians, social workers, chaplains, and aides
• Monitor patient condition and adjust care plans to maintain comfort

Help families navigate complex decisions with empathy and clarity

• Build meaningful, trusting relationships in deeply personal moments

Drive Quality & Compliance

• Participate in IDT meetings and care planning
• Support quality and compliance standards

Active RN license (or compact eligibility)

• Graduate of an accredited nursing program
• Minimum 1 year of clinical nursing experience
• Strong clinical judgment and compassionate care approach
• Experience in hospice, palliative care, or home health
• Case management and interdisciplinary care experience
• Experience with Homecare Homebase (HCHB)

Field-based care in homes, facilities, and community settings
