Advanced Registered Nurse

Join a collaborative healthcare team as a Nurse Practitioner (NP) or Physician Assistant (PA).

In this role, you will work on-site at a senior primary care clinic, providing personalized, high-quality care to adults aged 65+ with chronic conditions.

Responsibilities include managing patient health, conducting physical exams, screenings, and chronic disease management, and collaborating with physicians, pharmacists, behavioral health specialists, and care coaches.

You will spend meaningful time with patients, supporting their physical, emotional, and social wellness within a team-based care model.

Qualifications Include

Master’s Degree as an NP or completion of a PA program with board certification.

Active, unrestricted NP or PA license in Florida.

Active DEA license and Medicare/Medicaid provider numbers or ability to obtain them.

At least one year of experience in value-based, geriatric, or primary care managing adults 65+ with chronic conditions.

Proficiency in EMR systems.

Additional details:
The role involves full-time hours, with compensation ranging from $123,800 to $170,300 annually, plus potential bonuses.
