Licensed Practical Nurse

The Licensed Practical Nurse delivers nursing care to assigned patients in collaboration with the Staff Registered Nurse/Advanced Registered Nurse, and is accountable for patient care administered.

College Diploma in Nursing - Required

Two years Rehab and/or Medical/Surgical in the last five years preferred.

Documents all patient related clinical information and assesses patient for special needs and plans appropriate intervention. Comprehensive review of the patient's medical history is performed and documented.

Anticipates forecasted patient care discharge needs to ensure throughput targets met. Provides input that decreases inefficiencies for unit, staff in collaboration with the manager.

Contributes to formulation of nursing careplan for patients in consultation with the RN and in collaboration with the patient, families, healthcare members using established nursing diagnoses and nursing protocols. Prioritize nursing care needs of clients. Assist in the review and revision of nursing care plans with the RN to meet the changing needs of the patient.

Complies with regulatory guidelines for work area and professional practice standards. Assists with data collection, trouble shooting, and educating staff of performance improvement indicators. Utilizes available safety equipment and processes for patient care.

Care plan development and implementation with health care team members collaborating to develop long and short term patient goals. Communication with physicians using Situation, Background, Assessment, and Recommendation format. Administers medication utilizing the 5 rights and available safety equipment and processes. Utilizes clinical judgment with patient assessment and prescribed therapies to ensure promotion of goal obtainment. As a leader holds healthcare providers accountable for the standard of care.

Utilizes appropriate written communication, education packets and/or audio-video instructions. Credenza Health
