STAFF PHARMACIST - PRN/WEEKENDS

Job Summary
Provides leadership in optimal product preparation and dosing of medications, and provision of patient specific drug information to include patient specific dosing. Aides in the prevention and reporting of medication errors and adverse drug reactions according to policy. Supports quality relationships with health care professionals, families, and patients; and the pharmacist is actively involved in being further educated and providing education in all appropriate ways.
Position Highlights
Retail environment filling discharge prescriptions, completing refills for complex patient populations, setting up mail, and helping families of patients directly.
Clinical component includes evaluating doses and indications with their MAR, submitting prior authorizations, and making interventions with providers directly.
The position can vary among a variety of shift times to cover scheduling gaps and PTO.
Minimum 1 day/week or 2 days/every other week once fully trained and efficient, between the hours of 6:30am - 11:00pm.
Current needs are primarily midmorning to closing shifts.
Weekend rotation requirement of every 6 weeks with hours of 3:00pm - 11:00pm.
Key Responsibilities
Reviews drug orders and/or prescriptions to ensure the safe, appropriate and cost‑effective use of medications.
Provides outstanding patient care by optimizing medication preparation and resolving medication‑related problems.
Prepares and dispenses medications, provides drug information and manages medication therapy in accordance with federal, state and local regulations and hospital policy.
Supervises the activities of assigned support personnel and verifies the accuracy and appropriateness of all medications prepared by support personnel before dispensing.

Technical Capabilities
Pharmaceutical Quality Assurance & Compliance (Advanced): Possesses specialized knowledge of the appropriate rules and regulations and applies them in difficult, stressful and complex situations. Demonstrates expertise and advanced knowledge of process improvement and quality assurance techniques. Understands the principles upon which the rules and regulations are based. Demonstrates a commitment to continuous quality improvement and maintains compliance with federal, state and local regulations as well as VUMC policies and procedures. Able to explain rules and regulations that are ambiguous or unclear.
Pharmaceutical Computer Literacy (Novice): Has demonstrated a proficient understanding of computers or has successfully completed training for applications that are used to support medication use processes. Capable of using the fundamental features of pharmaceutical computer applications to support patient care issues of moderate difficulty.
Pharmaceutical Drug Preparation & Dispensing (Novice): Possesses sufficient fundamental proficiency to successfully review, prepare and dispense prescribed medications. Includes proficiency in the general principles of aseptic technique utilized to compound sterile preparations to minimize risk of contamination (where applicable).
Medication Management (Intermediate): Demonstrates proficiency in medication management in practical applications of a difficult nature. Capable of conducting medication management without requiring support. Determines the clinical appropriateness of each medication, dose, dosage form, indication, contraindications, potential adverse effects and interaction with concomitant medications. Able to monitor for therapeutic duplication or unnecessary medication, interpret laboratory results and provide drug information to other healthcare professionals, patients and families.
Mentoring & Coaching (Novice): Constructively trains and mentors others by providing positive direction and feedback. Provides constructive suggestions to improve situations, removes roadblocks and avoids judgment. Passes on experiences and job knowledge that can be learned easily.

Position Qualifications
License: Licensed Pharmacist - Tennessee.
Experience: Less than 1 year of relevant work experience.
Education: Bachelor's degree required.
Certifications: As required by regulatory bodies.
