Verovian Nursing Recruitment Agency is seeking a dedicated and compassionate Nurse Practitioner (NP) to join a Skilled Nursing Facility ( SNF ) care team in Glasgow, Kentucky . This full-time role offers the chance to deliver high-quality, patient-centered care to elderly and chronically ill residents in a supportive, collaborative long-term care environment.
Provide comprehensive care to patients in the skilled nursing facility (SNF), including assessment, diagnosis, and treatment planning.
Collaborate with interdisciplinary teams to develop and implement individualized care plans.
Educate patients, families, and staff about medical conditions, treatment options, and preventive care.
Perform routine medical procedures, such as wound care and diagnostic testing, within the scope of practice.
Ensure compliance with all regulatory and facility standards of care.
Act as a liaison between patients, families, and healthcare providers to facilitate optimal outcomes.
Current and unrestricted Nurse Practitioner license in Kentucky.
Master's or Doctoral degree in Nursing from an accredited program.
Certification as an NP (Adult, Family, or Geriatric focus preferred).
Minimum of 1-2 years of experience in a skilled nursing or long-term care setting.
