Care Management Nurse (LPN)

divh2Nursing Care Position/h2pTo provide nursing care to residents as prescribed by the physician and in accordance with standards of nursing practices and regulations./ph3Essential Job Duties and Responsibilities/h3pNursing Care:/ppPrepare and review medical records and nurses notes, and assess residents to observe and evaluate their physical and emotional status. Monitor resident activity, evaluate nursing care in accordance with professional standards, and provide nursing care according to physicians orders, care plans, established standards and facility policies. Coordinate and assist or prepare for new admissions, transfers, and discharge of residents. Transcribe physicians telephone orders. Communicate with residents, family members, other interdisciplinary team members and management regarding resident status./ppDrug Administration and Documentation:/ppPrepare and administer medications as ordered by the physician; in accordance with nursing standards and facility policies. Sign, date and perform all charting and record keeping in accordance with established policies and procedures./ppDirect Patient Care:/ppLead and monitor licensed and other professionals. may serve as team leader in this function./ppResident Rights:/ppUnderstand, comply with and promote all rules regarding residents rights./ppOther Job Functions:/ppCare Plans:/ppAssist and participate in developing and implementing a written care plan for individual residents that addresses the needs of the resident./ppStaff Development:/ppParticipate in in-service training, assist in implementing orientation programs and job skills training, maintains professional competence./ppSafety and Sanitation:/ppMonitor environment and care practices of nursing personnel to ensure compliance of established safety, fire and disaster, infection control and ensure all other departmental policies and procedures are followed; ppOther Duties:/ppOther duties as assigned/ppPhysical and Sensory Requirements (with or without the aid of mechanical device):/ppWorks in a climate-controlled and well-lighted environment. Works in an office and throughout the building daily. Has Category 1 risk of exposure to infectious diseases, but receives training in handwashing practices and Universal Precautions and has access to personal protective equipment if direct exposure is anticipated. Walking, reaching, climbing, bending, lifting, grasping, fine hand coordination, pushing and pulling, ability to distinguish smells, tastes and temperatures, ability to communicate with residents and employees, ability to understand and follow training, ability to instruct staff in service education, and the ability to remain calm under stress./ppMust be a Registered Nurse (RN) in good standing and currently licensed by the State. Experience as a Charge Nurse or supervisory experience preferred. Ability to convey, explain, or interpret complex specialized information related to long term care rules and regulations. Ability to build and maintain relationships, coordinate plans, distribute information and develop a mutual knowledge base for effective implementation of state and federal rules and regulations. Must be able to relate positively and favorably to residents, families, and government regulators.
