Sentara Home Care is seeking to hire an LPN Intake Coordinator.
Enter referral information in system including clinical information and orders for services. Develop and maintain a working knowledge of all services and resources provided by Sentara and available within the community. Anticipated Status/Shift:
Day Shift, Weekend hours as needed
Remote with occasional onsite meetings and trainings in Chesapeake, VA. Hourly Range: $17.31 - $28.80/hour
Trade School Graduate (Preferred)
LPN License - Active in Virginia (Preferred)

1-year of experience with clinical and customer service in a healthcare environment (Preferred)

• Care coordination experience (Preferred)
• Home Health experience (Preferred)
