We are seeking a compassionate, dedicated Registered Nurse to join our Assertive Community Treatment (ACT) Team and help individuals living with serious and persistent mental illness achieve greater stability, wellness, and independence.

About the ACT Team The Assertive Community Treatment (ACT) Team provides comprehensive, community-based behavioral health services to individuals with serious mental illness and co-occurring disorders. Our team works collaboratively to ensure clients receive the support, treatment, and resources they need to successfully live, work, and thrive in their communities.

As an ACT Team RN, you will play a vital role in coordinating healthcare services, managing medications, supporting wellness, and advocating for individuals throughout their recovery journey.

What You’ll Do As a Registered Nurse on the ACT Team, you will:

Clinical Nursing Care

Conduct comprehensive psychiatric, clinical, and physical health assessments

Monitor client health status and treatment progress

Maintain medication records and ensure regulatory compliance

Complete nursing interventions, screenings, and health monitoring activities

Community-Based Support

Provide nursing services in homes and community settings

Promote wellness, disease prevention, and health education

Care Coordination

Coordinate services with physicians, psychiatrists, specialists, pharmacies, and hospitals

Assist clients with accessing healthcare and community resources

Facilitate referrals and follow-up care

Build strong partnerships with healthcare providers and support systems

Respond to psychiatric, behavioral health, and medical crises

Collaborate with psychiatrists, therapists, case managers, and peer specialists

Provide clinical recommendations and nursing expertise to the treatment team

Maintain compliance with nursing standards, HIPAA requirements, and agency policies

Support infection control and quality improvement activities

Support and train staff on medication administration and health monitoring

Advocate for client rights, access to care, and community integration

Current and unrestricted Registered Nurse (RN) license in the State of Delaware

Graduate of an accredited School of Nursing

Ability to obtain and maintain Delaware Department of Correction security clearance

Valid driver's license, current vehicle registration, and reliable transportation

Bachelor of Science in Nursing (BSN)

Minimum of one year of nursing experience

Experience in behavioral health, community mental health, ACT programs, substance use treatment, or integrated healthcare settings

Experience providing community-based services and care coordination

Full-Time Position

Community-Based Role with Local Travel

Flexible schedule based on client needs

Participation in on-call rotation as required

Regular travel throughout the service area is required.

Ability to drive regularly for community-based visits

Ability to respond effectively during emergency situations
