Home Care LPN

Works under the supervision of the Nurse Manager. The Staff LPN's primary job function is to make home visits to render nursing care in accordance with the physician's plan of care and assignment. Completes clinical records on each visit. Accountable to the scope of practice outlined by the Ohio Board of Nursing under the direction of the registered nurse. participates in weekend and holiday rotations. Occasionally travels 50-75 miles/day with 5-6 skilled nursing visits/day. Mileage paid per mile / company car if eligible.

High School Diploma or successful completion of an equivalent High School Exam required

Graduate from an accredited school of practical nursing required

Licensed to practice as an LPN in Ohio required

Licensed to practice as an LPN in Kentucky to be obtained after hire required.

Prior home care experience preferred

Approximately one year prior medical/surgical experience required.

Makes home visits to render professional nursing care in accordance with physicians plan of care and assignment.

Completes clinical records on each visit. Communicates changes in patient status to patients, family, physicians and other health care team members.

Teaches the patient, family or significant others health restoration and maintenance procedures. Documents patient response in the clinical record.

Always reports adverse reactions and abnormal signs and symptoms to the physician in a timely manner.

Makes recommendations to RN Case Managers for alterations in the plan of care.

Pre-cleans, decontaminates and transports contaminated instruments according to policy
