Advanced Practice Nurse- ICU

Job Summary Responsible for delivery of patient care through the nursing process of assessment, planning, implementation, and evaluation. Directs and guides patient teaching and the activities of ancillary personnel while maintaining professional nursing standards. Adheres to GCMC’s Compliance Standards, Corporate Compliance Policy, Code of Conduct, and Conflict of Interest Policy. Keeps abreast of all pertinent federal, state, and GCMC regulations, laws, and policies as they exist and as they change.

Experience Entry level – graduate per education requirements

Specialty Units – at least 1 year of medical‑surgical nursing preferred (e.g., ICU, Maternity, Psych)

Skills See core competencies and unit specific competencies.

Licensure / Certificates / Certification Current NYS license to perform as a registered professional nurse

Current BLS‑CPR certification required (or within first 30 days of employment)

Specialty Unit – Maternity – Neonatal Resuscitation Certification within 6 months of employment

Specialty Unit – ICU – ACLS within 6 months of hire

Essential Job Functions Assessment – Performs patient assessment and documents accurately and timely per nursing and department policy.

Patient Monitoring – Monitors patient condition, vital signs, and safety per protocol; reports changes to charge nurse or physician; initiates CAT team if criteria met; follows National Patient Safety Goals.

Medication Administration – Safely administers medications, IV fluids, and blood products per policy; uses e‑Mar, barcoding, and scanning with at least an 85% scan rate; maintains IV site and dressing per policy; logs blood product administration; secures electronic signature with Omnicell.

Patient and Family Education – Assesses educational needs of patients and families; documents needs; provides teaching sheets; evaluates understanding post‑teaching.

Safe Discharge / Discharge Planning – Contributes to or initiates discharge plan on admission day; completes all pre‑discharge checklist items; gives correct forms and instructions using two patient identifiers; ensures follow‑up information and community resources are available.

Assist Providers – Assists physicians/producers with bedside procedures; remains competent with unit‑specific procedures; reads back critical values and phone orders; uses SBAR for reports.

Equipment Use – Demonstrates knowledge and safe use of equipment, including IV pumps, alarm settings, patient lift equipment; follows hospital policy.

Nursing Clinical Skills – Performs nursing treatments and procedures safely; conducts hourly rounds using the 4 “P’s”; uses patient room whiteboards for communication.

Emergency Preparedness – Demonstrates knowledge and actively participates in emergency preparedness plans and operations.
