Shift length: 12 hour shifts (36 hours per week)

Job Responsibilities:

Work 36 hours per week with 12-hour shifts.

Coordinate scheduling, referrals, and follow-up care with clinical and administrative teams.

Administer ordered therapies and monitor for adverse reactions or changes in condition.

Document care delivered, patient response, and communications in the medical record.

Perform medication reconciliation activities and communicate discrepancies.
