Home Health Aide
You'll join a compassionate, fast-growing home health and home care team that supports patients one-on-one in their homes and communities. Provide personal care support including bathing, grooming, dressing, oral hygiene, and toileting

• Assist with mobility, transfers, ambulation, and range-of-motion exercises per care plan
• Maintain a clean, safe, and orderly client environment
• Use safety equipment properly (rails, supports, stools, etc.)
• Document care provided according to company and regulatory standards
• Travel reliably to patient homes and assigned visits across your service area
• Complete required in-service training, including annual dementia education

Accurate, timely care documentation • Positive feedback from patients and families

• Completion of required certifications and training on schedule
• Strong communication with supervising nurse and care team

Active HHA or CNA certification or 6 months experience as a caregiver either professionally or personally

• Ability to follow written care instructions and document services
• Reliable transportation and ability to travel to client homes
• Willingness to complete TB testing, background screening, and onboarding requirements

Home health or home care experience

• CPR and Adult First Aid certification
• Experience working with dementia or memory care patients

One-on-one care in patient homes and occasional facility settings

• Flexible scheduling options may be available
• Travel within your preferred coverage area
