Licensed Vocational Nurse (LVN) - Home Health and Hospice Care

Provides comprehensive home health nursing care by coordinating and delivering patient treatments according to physician orders, agency policies, and nursing best practices. Responsible for evaluating patient needs, implementing care plans, and supervising nursing assistants to ensure high-quality home-based care.
Coordinate and conduct home health visits, including start of care, wound care, and follow-up assessments.
Supervise and evaluate home health aides and licensed practical nurses/licensed vocational nurses to ensure compliance with care plans.
Travel up to 75 miles from the assigned care center to provide patient care across the designated territory.
Maintain proficiency in HomeCare HomeBase (HCHB) and Oasis, ensuring all documentation meets agency standards.
Hold a valid RN license in the state of assignment with no pending licensing issues.
Possess a minimum of 1 full year of recent adult home health or hospice experience.
Be willing to travel up to 75 miles and accept the physical demands of home visits.
Access to ongoing training and professional development opportunities in home health care.
Flexible scheduling options with potential to expand clinical skills within the home health setting.
No hospital or facility names included.
Healthcare providers must adhere to industry standard travel and documentation policies.
