Partner with care team members to develop and implement patient-centered clinical plans.
Deliver wrap-around services through home visits and collaborative care alongside providers and RNs.
Perform key clinical tasks in the fieldchronic condition support, wound care, phlebotomy, medication review, and more.
Engage hard-to-reach patients, bringing them vital care and support directly at home.
Ensure continuity of care by providing coverage for team members when needed.
Help drive better patient outcomes, lower costs, and greater patient and staff satisfaction through our interdisciplinary care model.
Licensed Practice Nurse, unencumbered license certification in the state of employment
Experience providing care for the whole person, including caring for adults and seniors with high medical and social complexity
Intermediate to advanced computer and software skills, including Patient Health Exchange Platforms, Electronic Health Records, and Outlook
Ability and means to travel as needed in a timely manner to locations that may have limited access to public transportation; No vehicle-related suspensions, DUI, reckless driving, leaving the scene of an accident, vehicular manslaughter, felony convictions, or 3 or more moving violations within the last 5 years. Medical or religious exemptions will be considered contingent upon the review of appropriate documentation.
