LPN Ambulatory / Care Management Nurse

Provides comprehensive outpatient nursing care to adult and pediatric patients within a clinic setting, focusing on patient assessment, care planning, education, and coordination. Acts as a key member of the healthcare team to promote health, manage chronic conditions, and support patient self-management.

Develop and implement individualized care plans based on patient assessments and medical directives.

Provide direct patient care including triage, pain management, and health education.

Supervise and collaborate with Licensed Practical Nurses, Medical Office Assistants, and other clinical staff to ensure quality care delivery.

Document patient interactions, care plans, and follow-up activities accurately and promptly.

Coordinate with external healthcare providers to ensure continuity of care from admission to discharge and referrals.

Minimum of 2 years outpatient or urgent care experience in adult and pediatric populations.
