About the Opportunity We're seeking a dynamic, compassionate Community Liaison to drive growth and build meaningful partnerships across our service region. In this key role, you'll be the face of our organization, establishing trust-based relationships with physicians, healthcare partners, and referral sources while helping families access quality end-of-life care services.
This position combines relationship-building, outreach, and advocacy. You'll work directly with hospital discharge planners, skilled nursing facilities, physicians, and social workers- conducting regular visits, educating the community about our services, and facilitating conversations with patients and families during critical moments.
What You Do Build and nurture relationships with key decision-makers in the medical community
Conduct consistent outreach to drive awareness and preference for our services
Generate referrals through targeted engagement with healthcare professionals and community agencies
Serve as a knowledgeable advocate for our mission and specialized services
What You Bring Bachelor's degree in a related field (or equivalent healthcare/sales experience)
Valid driver's license, reliable transportation, and auto insurance
Hospice or palliative care experience preferred
