Grandview Nursing & Rehabilitation
At Grandview Nursing & Rehabilitation, we are seeking a dedicated and compassionate Hospital Liaison to join our team. As a vital member of our staff, you will play a crucial role in fostering strong relationships between our center and local hospitals, ensuring seamless transitions for our patients and families. Develop and maintain relationships with local hospitals, healthcare providers, and other stakeholders to ensure effective communication and collaboration
Participate in discharge planning and care conferences to ensure that patients receive comprehensive care
Maintain accurate records and reports, ensuring compliance with regulatory requirements and facility policies
High school diploma or equivalent required; degree in a related field (e.g., experience as a liaison or care coordinator a plus
