Licensed Practical Nurse Home Healthcare Full Time

Position Overview We are seeking a dedicated and compassionate nursing professional to provide high-quality patient care in a supportive clinical environment. In this role, you will work collaboratively with physicians, registered nurses, and other healthcare team members to deliver safe, effective, and patient-centered care. Provide direct nursing care in accordance with care plans, policies, and established clinical standards.

Monitor patient conditions, document observations, and report changes to the appropriate healthcare team members.

Assist with patient assessments, vital signs, wound care, specimen collection, and other routine clinical procedures.

Educate patients and families on care instructions, health maintenance, and follow-up needs as directed.

Collaborate with interdisciplinary teams to help ensure coordinated, high-quality care.

Follow infection control, safety, and regulatory guidelines at all times.

Current licensure as a practical nurse in the applicable state or jurisdiction.

Completion of an approved practical nursing program.

Ability to provide safe, compassionate care in a patient-focused setting.

Basic knowledge of nursing procedures, infection control, and patient safety practices.

Commitment to professionalism, confidentiality, and ethical care.

Previous experience in a hospital, clinic, long-term care, assisted living, or similar healthcare setting.

Familiarity with electronic health records and clinical documentation systems.

Experience supporting patients with chronic conditions or complex care needs.
