As a Home Health Caregiver with Modani Care, you will have the opportunity to make a meaningful difference in the lives of our clients. Your primary responsibility will be to provide personalized care and support to bed-bound patients in the comfort of their own homes. Your expertise and dedication will enable our clients to maintain their independence, dignity, and quality of life.
Provide direct patient care, including assistance with daily living activities, mobility, and positioning
Develop and implement personalized care plans in collaboration with clients, families, and healthcare professionals
Monitor and report client progress, identifying opportunities for improvement and adjustments to care plans as needed
Maintain accurate records and documentation of client care and services provided
Collaborate with the healthcare team to ensure seamless transitions and continuity of care
Opportunities for professional growth and development through ongoing training and education
Minimum of 2 years of experience in home health care or a related field
Ability to lift, transfer, and reposition clients with care and attention to safety and dignity
Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare professionals
