With a focus on geriatric care and geriatric rehabilitation, our 142-bed facility offers both skilled nursing and long-term care services. LPN Duties and Responsibilities

Perform ongoing nursing assessments and monitor residents’ health and well-being

Supervise, mentor, and guide CNAs and other direct care staff in daily resident care

Administer medications and treatments safely and accurately, following care plans

Delegate appropriate tasks to team members while ensuring quality of care

Communicate changes in residents’ condition and treatment effectiveness to the nursing team

Educate residents and families about health, wellness, and care plans

Support household operations and maintain a safe, comfortable environment

Carry out additional duties as assigned by the Nurse Manager or leadership team

LPN Compensation and Schedule

Pay Range: $30.00-$34.00 per hour

Competitive shift differentials: +$1.50/hr for weekday B & C shifts and +10% more/hour for all weekend shifts

Available Shifts: Day (7:00am – 3:00pm), Evening (3:00pm – 11:00pm), Night (11:00pm – 7:00am)

Full-Time (37.5 hours per week), Part-Time (22.5 hours per week), Weekend availability required

Sign-On Bonus: $3,000! for Full-Time LPNs

LPN Qualifications and Skills

Current NYS Licensed Practical Nurse (LPN) license at time of hire

New graduates encouraged to apply

Experience in geriatric, rehabilitation, or long-term care preferred

Flexibility to meet the needs of residents and the household

Availability for rotating weekends and holidays

Strong computer skills and accurate documentation abilities

Ability to safely perform essential nursing functions (reasonable accommodations considered)
