As a Nurse Navigator (RN), you will serve as a patient's guide through the healthcare system, helping coordinate care, remove barriers, and improve the overall patient experience.

Serve as the primary point of contact for patients and families

Coordinate appointments, testing, treatments, and referrals

Assess barriers to care and connect patients with community resources and support services

Advocate for patients and help ensure timely, appropriate care across the continuum

Provide hands‑on patient care including but not limited to vitals, patient intake, in‑clinic procedures

Cover other clinics as necessary
