Join a mission-driven team making a real difference in community health! We are a busy, fast-paced Federally Qualified Health Center (FQHC) seeking a dedicated Licensed Practical Nurse (LPN) to support our Hartford site and mobile health unit.
No nights or weekends
Clinic-based care plus outreach on our mobile van

The LPN is responsible for delivery of clinical care and works closely with medical providers, medical assistants and support staff to ensure the effective and efficient delivery of primary health care services to all patients.
Graduation from an accredited licensed practical nurse program required. Associates in a related field preferred. Must have 2 years of clinical experience providing nursing services to patients from diverse economic and cultural backgrounds, as well as exceptional communication, interpersonal and organizational skills are required. Outpatient primary care settings experience preferred. Bi-lingual English/Spanish a plus.

Works under the supervision of a Registered Nurse, and performs orders given by the medical provider.
Provides direct care to patients, carrying out orders given by the medical provider, including obtaining patient medical histories, obtaining vital signs, performing blood draws, preparing patients for and performing diagnostic testing, assessing and providing wound care, dressings, administering injections and assisting medical provider in the examination room.
Contributes to a collaborative work environment by initiating open communication, a positive intent and offering assistance to support patients, families and care givers.
Respects patient and family cultural, spiritual and personal norms by acknowledging the essence of each person and appreciating and responding to the uniqueness of each person.
Provides chronic care disease support to the patients by phone, in person and in all aspects of the care, treatment and services provided and in accordance with the clinic's integrated model of care. Develops patient focused self-management goals and care plans with the patient.
Ensures gaps in care for screenings, chronic care tests, (such as HgbA1c for Diabetics), health literacy assessments, tobacco screenings, BMI, etc. Demonstrates strong nursing knowledge base and skill in job performance.
Assumes responsibility and accountability for coordinating care, in collaboration with the medical providers.
Ensures that continuity of care is maintained in all aspects of patient care.
Contributes to medical staff's effectiveness by identifying patient care issues that must be addressed; and implementing medical providers' directives.
Serves as liaison to medical providers.
Protects patients and employees by adhering to infection-control policies and protocols.
Documents timely and accurately patient care services in the electronic health record. Tracks referrals, labs and diagnostics in accordance with policy in the electronic health record.
Participates with regular staff meetings focused on coordinating care within an interdisciplinary team.
Performs job responsibilities consistent with standards for best practice nursing, including excellent communication with all providers to ensure the delivery of high quality care.
Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological approaches, skills, and techniques that reflect an understanding of culture.
