Registered Nurse for Hospice Care

Tri-State LifeCare Registered Nurse
The TSLC RN is responsible for conducting patient assessments, enrolling patients in Tri-State LifeCare, following the plan of care, providing treatment and working in conjunction with the members of the team in order to ensure comfort and quality of life. Performs assessment of assigned patients and derives nursing diagnoses based on assessment data.
Plans nursing interventions in collaboration with the interdisciplinary team to resolve current problems or prevent potential problems.
Provides routine nursing visits and personal care while following the plan of care:
Provides comfort oriented physical care to patients.
Provides and documents teaching to patients and/or families to assist in the patient's comfort and records the response of the family and/ or family
Demonstrates the physical care needed for the patients, family or Hospice Aide

Proficiency with scheduling software
Education and Work Experience:
Minimum of 1-year patient care experience as a registered nurse.
