Schedule: Full-Time Compensation: $75,000 $110,000 annualized (per-visit model based on volume)
Location: Greater Twin Cities Metro Area (21-county service area)

We operate alongside Interim Healthcare, Lifespark, Adara, Aveanna, and Gentiva, but our focus is simple. Support the nurse so the nurse can focus on the patient.
Provide skilled nursing visits in homes and community settings.
Perform assessments, wound care, and medication management.
Educate patients and families on care plans and recovery.
Collaborate with RN Case Managers and interdisciplinary teams.
Active Minnesota Registered Nurse (RN) license.
Experience in home health preferred, but not required.
Comfortable in a field-based, travel-focused role.
