Therapy - Physical Therapist - PT

Home Health Physical Therapist

This is a 13-week contract position.

Monday-Friday, 5 shifts per week, 8 hours per shift.

We are currently seeking a Home Health Physical Therapist. The ideal candidate will provide high-quality, patient-centered therapy services in home settings. Responsibilities may include evaluations, treatment plan development, and implementation of therapy interventions.

Responsibilities:

Conduct patient evaluations and develop individualized treatment plans.

Implement therapy interventions and monitor patient progress.

Document patient care and progress accurately in Homecare Homebase.

Participate in case conferences and collaborate with interdisciplinary teams.

Evaluate and adapt treatment plans based on patient needs and outcomes.

Required Experience / Certifications / Licensure:

Active Physical Therapist license.

Experience in home health and OASIS documentation is required.

Homecare Homebase documentation experience preferred; candidate may be considered for reduced rate training for the first two weeks.
