Rehab Liaison, full-time, RN required

Community Regional Medical Center in Fresno seeks a Registered Nurse (RN) Rehabilitation Clinical Liaison to join Signet Health in partnership. The role centers on pre-admission assessments and on-site care coordination outside the hospital walls.
You will build relationships with referring physicians, case managers and hospital teams, and will communicate referrals and discharges with medical leadership. You may transition to a full-time role with benefits; the position pays $80/hour.
